The latest modest proposal
State transportation departments should pay insurance companies to defray the costs of long term care of patients in their state. The more chronic heart and lung disease occurs in a state, the more a state DOT should defray the costs of of what it helped to create. It can get a credit for this obligation if it funds transit or complete streets projects.

Doesn't sound terribly different from the plans where Utility companies rebate the purchase of insulation and more efficient appliances because it is cheaper than building new power plants.
